How do I obtain a copy of my Ambulance Bill?

Our goal is to help you obtain the information you are looking for in the simplest way we can so that you can focus on healing or caring for your loved one.  The information below should answer most of your questions or connect you with the resources you are looking for.  Of course, if you can't find what you need, please contact us so that we may assist you.

Patient privacy isn't only the law, it is something that we take very seriously.

 

For that reason, we have implemented a process that must be followed in order to obtain a copy of this information.   These steps, while may be considered cumbersome are required to assure that the release of Protected Health Information conforms to our policy and the law.

Attorney's Offices: most commonly request copies of EMS Reports and Billing Statements on your behalf.

  • Attorney may call 888-987-2054 for assistance.  When contacting our representatives, the caller will be greeted with an auto-attendent.  The first set of options presented will not apply for an Attorney.  Please wait a for the first set of prompts to end, there will be a few moments of silence and then a new set of options will be presented.  From that list, please select Option #4 to select Attorney.  This will route the call to the appropriate individuals and allow for efficient handling of these types of requests.  
  • Attorney's may also fax their requests (with all industry standard authorizations) to 614-987-2075.

Patients and Authorized Patient Representatives.

  • Patients may call 888-987-2054 for assistance.  Please have your account number available to enter if you have it.  Your account number should appear on the top right corner of your letter.  If you don't have your account number, that isn't a problem.  Simply remain on the line and our team will assist you.

Payments for Fire/Emergency Services (Structure Fires, Motor Vehicle Fires, Motor Vehicle Crashes, Hazardous Material Spills, Releases & Dumping and so on:
